Description:
Transitions helps women overcome the key barriers to successful, long-term recovery by addressing parenting and family bonding issues, self sufficiency, education, and job skills as well as the addiction. The program consists of two phases: Residential Treatment is the initial phase in which the women focus on addiction issues and assume responsibility for their substance abuse and its consequences. The purpose of this phase of treatment is to provide a therapeutic environment to identify and attend to the emotional and physical needs of addicted women. Family strengthening and reunification is a major focus of this phase. Towards the end of residential treatment, Transitions works to encourage and assist the women in establishing independence and self-sufficiency by attending school or job training, obtaining employment, and applying parenting skills. Residential treatment may last up to one year. Community Housing is a six-month phase in which families move out into the community with regular home visits with their case manager. They return to Transitions for scheduled relapse prevention groups and support.
