Details: It was in 1983 that Dr. Jose E. Hernandez was presented with a mission that was greater than he, alone, could fulfill. Thirty years later, that mission is known as the Agape Network which includes South Florida Jail Ministries and AGAPE Family Ministries, a comprehensive community health center. "Agape," meaning “UNCONDITIONAL LOVE†is one of the first non-denominational and non-sectarian faith-based Substance Abuse, Mental Health Residential Treatment Facilities and Community Health Centers in South Florida. The incorporation of spiritual health into the healing process is what separates AGAPE from other organizations. Expanding its services beyond its residential facility, the AGAPE Network has become a comprehensive integrated health care system that treats the whole person and family. Agape helps bring true change in which individuals improve their health and wellness, live a self-directed life, and strive to reach their full God-given potential. In partnership with the Health Choice Network (HCN), a national model of a successful collaboration among community health centers and providers, HCN/AGAPE is nationally acclaimed for the meaningful use of their information technology infrastructure. Included, is an integrated system of care that combines and centralizes the electronic health records, finance, billing, practice management, research, informatics and reporting functions of the entire network and agency. Other partners in this venture include Community Health of South Florida (CHI), Citrus Health, and others, with over 200 sites throughout Florida serving more than 500,000 patients per year. HCN and its national partners are helping to create a regional network that will link providers, hospitals, clinics, laboratories, pharmacies, and public health institutions. Ultimately, this will improve quality and consistency of care, tracking, sharing, research, and reporting best practices in successful patient/client outcomes. With this amazing network, Agape is able to provide a medical health home with a broad-ranging continuum of care that is holistic and client centered. Agape integrates behavioral health care with primary care, empowering individuals and families to self-manage their condition and make better use of healthcare and social services resources.
