Description: Community Bridges, Inc. (CBI) was incorporated as a private non-profit, 501(c)(3) organization in 1982 and has a 31 year history of providing comprehensive, medically integrated behavioral health programs which include prevention, education and treatment services using cutting edge, nationally recognized treatment models throughout Arizona. Today, we are one of the largest statewide providers offering fully integrated medical and behavioral health care in 14 communities in Maricopa, Pinal, Gila, Yuma, Navajo, Apache and Cochise Counties by providing a continuum of care that begins with prevention and continues for individuals and families through treatment and recovery. During the past four years, CBI has transformed recovery of those we serve holistically and more effectively, by delivering direct physician and nurse practitioner services, both on site in Maricopa County, and through telemed to each of our service locations throughout Arizona. CBI operates 29 programs throughout Arizona that are all licensed by the Arizona Department of Health Services Division of Behavioral Health. Our prevention and clinical programs are accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF). Early in 2012, CBI received another 3 year accreditation cycle, adding 14 NEW clinical programs to our list of services. CBI’s Rural Level IV Stabilization and Recovery Units (SRU) are located in Payson, Globe, Winslow, Holbrook, Benson and Casa Grande. These licensed facilities are funded by Cenpatico Behavioral Health and NARBHA. The primary function of these facilities are to provide crisis intervention, triage, stabilization & short-term recovery services (three to five days) to individuals who present having had used a mind altering substance within the last 7 days.

Crisis stabilization services are emergency interventions that can last up to a maximum of 23 hours. Services include medically monitored stabilization, triage, evaluation, assessment, and transitional services for individuals who have either used a substance within the last five days, or are currently experiencing a substance or co-morbid related crisis. Our inpatient medical detoxification centers utilize physician-directed protocols (generally three to five days) with nationally recognized Best Practice approaches. These medical protocols are directed by licensed medical practitioners and are supported by our clinical team of registered nurses (RN), behavioral health specialists, peer support specialists, and transition coordinators. Upon completion of the program, our clinical team develops a comprehensive discharge plan with each individual that takes into account all six dimensions of the American Society of Addiction Medicine in order to maximize successful transition to the next level of care.

Description: NCADD has been serving the Greater Metropolitan Phoenix area since 1960. Originally NCADD provided family assessments and referred people to community based services. In 1986, the agency began using our expertise to provide direct treatment services to women in need. In 2003, Weldon House was opened, providing each family with a 2 bedroom apartment (a home) where they can learn to be a family and where children have the opportunity to live normal and stable lives. In July 2007, NCADD opened Healthy Connections. The name was chosen to promote hope, not judgment. Healthy Connections is an intensive case management program that links pregnant substance abusing women to services and resources that will give them the foundation to deliver drug/alcohol free babies and establish a healthy lifestyle for their families and the community. May 1, 2011 was the addition of Legacy House, emergency housing for pregnant women with substance abuse. January 2010 Sustain Work Adjustment Program became operational. Work adjustment programs typically in the past have been focused on assisting mentally and physically disabled to obtain the skills needed to enter the work force. NCADD’s work adjustment program utilizes best practices that are making a break through within the treatment of substance abuse. We strongly believe in "gender specific" treatment. When addressing core issues/trauma, women need to feel safe and have the atmosphere to do so. A unique difference that Pia's Place has from other facilities, is that we admit no more then 20 women at a time. This allows each client to create a specialized treatment plan. Our therapists use a wide variety of treatment models and therapeutic techniques. CBI operates 29 programs throughout Arizona that are all licensed by the Arizona Department of Health Services Division of Behavioral Health. Our prevention and clinical programs are accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF). Early in 2012, CBI received another 3 year accreditation cycle, adding 14 NEW clinical programs to our list of services. Crisis stabilization services are emergency interventions that can last up to a maximum of 23 hours. Services include medically monitored stabilization, triage, evaluation, assessment, and transitional services for individuals who have either used a substance within the last five days, or are currently experiencing a substance or co-morbid related crisis. Our inpatient medical detoxification centers utilize physician-directed protocols (generally three to five days) with nationally recognized Best Practice approaches. These medical protocols are directed by licensed medical practitioners and are supported by our clinical team of registered nurses (RN), behavioral health specialists, peer support specialists, and transition coordinators. Upon completion of the program, our clinical team develops a comprehensive discharge plan with each individual that takes into account all six dimensions of the American Society of Addiction Medicine in order to maximize successful transition to the next level of care.
